Located in Sun Prairie, WI, Perinatal Home Visiting Programs offer two essential services, the Nurse-Family Partnership and Prenatal Care Coordination, designed to provide support for expectant individuals in Dane County. Our programs are dedicated to ensuring that everyone, regardless of legal residency, has access to the resources needed for a healthy pregnancy and baby. Our compassionate nurses are here to help you set and achieve personal goals, navigate healthcare options, and gain valuable knowledge on nutrition, childbirth preparation, safe sleep practices, child development, and breastfeeding. Enrollment in the Prenatal Care Coordination program is open throughout your pregnancy, while the Nurse-Family Partnership is available for first-time parents up to 28 weeks of pregnancy, providing support until your child is two years old. We welcome those who meet income eligibility through programs such as WIC, FoodShare, Medicaid, or BadgerCare Plus.
