Located in Lost Creek, WV, the Living with Type 2 Diabetes Program is a welcoming initiative designed for individuals recently diagnosed with type 2 diabetes. This free, bilingual program offers a comprehensive 12-month journey that enables participants to understand diabetes and develop effective management strategies at their own pace. Enrollees benefit from six informative e-booklets tailored to address various aspects of diabetes care, alongside a monthly e-newsletter filled with seasonal tips and resources. Participants also gain access to local community events and have the opportunity to ask their diabetes-related questions during our monthly 'Ask the Experts' Q&A series, facilitated by ADA experts, which can be attended online or by phone. Through this program, we provide valuable tools, insights into the connection between diabetes and heart health, and exciting giveaways, ensuring that everyone has the support they need to thrive with diabetes.
