Badly want my academic comeback!!

I am the second year student of computer science engineering, I am facing so much lack of focus towards my studies and I constantly avoiding my studies like whole month. It's leads to the stress and anxiety during exams. It's happening since first semester now I am in third semester. It's feels vicious cycle. Now I want solution to break this cycle and want to study Hard. Please help me !!
