My own worst enemy

I just recently returned to work after being unemployed for 6 months and I am not performing to the level I should be. I frequently give up shifts, ask to leave early and sometimes even come to work intoxicated (I do not work anywhere that I can endanger people). It seems I’m pushing my limits and can’t seem to get my life back in control. I enjoy the job but my depression and anxiety from being out of work for so long previously is seriously taking a toll on me and I’m afraid if I can’t get it together I will lose this job as well. How do I take back control of my own life from myself?!
