How to socialize when you're disabled?

The title is self-explanatory.

My main chronic condition keeps me at home sometimes. It can do so for days when I have a flareup. Currently I don't own a car, in part because of my condition...am worried that when I drive, I may not always be safe. I rely on alternative transportation when necessary.

On top of that, I have nocturnal tendencies...& am old enough that the usual nightlife does not appeal to me anymore. Bars are boring; drinkers can be & sometimes are very irritating; I don't like loud music &/or noisy environments.

So with all the above, where does that leave me? What can I do? I don't need a lot of friends...just two or three decent ones would do. But meeting people has never been easy for me; these days it's harder than ever.
