Taking a step back to dating with my SO to improve some of my shortcomings
I need advice on how to take more initiative in the relationship. Being supportive of someone with chronic illness and grad school responsibilities. Lastly with dealing with being clingy. How do you get over wanting to spend a lot of time with you So?

While we are not able to provide specific advice here as you are the best person to make those decisions based upon your situation, it can help to hear other perspectives and experiences.
From what you've shared it sounds like your SO is currently occupied in managing their chronic illness and grad school responsibilities. Perhaps you both get less time together than you'd like.
I'll try to share a few things that can help when one wants to spend more time with someone who might be busy with other commitments. The extra time you want to spend together, but is being spent alone, can be used to plan and build the moment which you will spend together. e.g., it can take 2-3 hours to plan, prep and cook a beautiful meal which then needs only 20-30 mins together to enjoy it. Similarly ironing, organizing stuff, gardening are all time/energy consuming tasks that can create gifts of love depending on how much thought and planning is put into it.
Takes time and energy to build a memorable moment.
