Recently, I’ve started confronting some of my mental health issues and it’s like I’ve been awakened almost. But me being in my struggles may have ruined my relationship with my SO. She’s planning on moving out, to be fair after 5 years of my mood swings, I probably would want to leave me too. I’ve tried to communicate to her that I’ve been struggling, that those moments aren’t me, but all seems lost, she’s planning on leaving soon. I didn’t even know I was struggling until recently, but she tells me I should have realized it earlier. I just don’t know what to do, I’ve even conveyed to her I’m looking for help. That this isn’t some flash in the pan change that I’m going to get better. I just want to save this. But I’m at a loss. I may have ruined the one last important thing to me.

Update: I’ve began the process of seeking professional help with my issues, I can’t cope if I don’t know what’s wrong right?

Hey, I',m sorry that you're going through that.

It must be very difficult for you to acknowledge the fact that an important person wants to leave you in such times. You realizing that you are going through things is itself a ray of light in your life, many fail to recognize that. We are different people and we function at our own pace, it takes time for certain things to show up and that's completely okay. I know that person leaving you is something that's causing you even more distress, such are the times when we need their support more than ever. Let me tell you that we're here for you and It's really good to know that you've start seeking professional help, you're on the right path. I hope things get better for you soon. Feel free to message me in case you feel like talking about it :)
