How to get graduate badge ?

Hi

Can anyone please let me know how to get the graduate badge ? Do I have to fill any application form to apply for the graduate badge ?

Yeah, I completed it long back. Is there anything else I should do to get the graduate badge ?

Upon completion it's peer reviewed, if you don't have the badge, you need to take it again.
