The Gatekeeper Project, located in El Monte, CA, is dedicated to addressing health disparities faced by older adults who may be isolated, economically disadvantaged, or living with disabilities. Through a compassionate network of trained volunteers, we offer essential, non-medical services aimed at improving the well-being of this underserved population. Our program begins with a comprehensive in-home initial needs assessment, followed by the development of an individualized service plan and ongoing case management to ensure that every older adult receives the support they need to thrive.
