In Elk Grove, CA, Meals on Wheels by ACC is dedicated to providing warm, nourishing Home-Delivered Meals to seniors who are unable to leave their homes due to illness, disability, or recovery. Our mission is to ensure that no senior is left without adequate nutrition, offering daily hot meals delivered Monday through Friday, along with a weekly box of five frozen meals for reheating. Each new participant will receive an in-home visit from a caseworker who will assess eligibility, explain the program in detail, and share valuable information on health and safety topics, including fall prevention. We conduct reassessments every three months to ensure that our services continue to meet our clients' evolving needs.
