The ACEs Network of Care Clinical-Community Linkages Program in Fresno, CA, is dedicated to connecting Central Valley providers screening for Adverse Childhood Experiences (ACEs) with trauma-informed services. Our compassionate team of Community Health Workers (CHWs) educates patients about their ACEs scores while helping them navigate a comprehensive suite of community resources, including healthcare, medication assistance, behavioral health services, and social services. By identifying patients' stressors and enhancing their support systems, we strive to foster safe, stable, and nurturing environments for those with high ACEs scores. Our program facilitates valuable referrals for food access, housing, transportation, and health education workshops on chronic diseases and wellness. Recognizing the urgent need for trauma-informed care, we work collaboratively with clinicians and community leaders across California, inspired by the pioneering initiatives of the Office of the California Surgeon General and the ACEs Aware Initiative, to empower our community with innovative and effective solutions.
