Based in Oakland, CA, the Wellness Program at Project Open Hand is dedicated to providing food assistance to individuals facing medical challenges. Our mission is anchored in the belief that nutritious food is essential for healing and well-being. We support our clients with medically-tailored meals and fresh groceries designed specifically for those living with critical illnesses. Our Registered Dietitians conduct comprehensive wellness assessments to ensure that every meal aligns with individual dietary needs, while also offering ongoing nutrition counseling and support. By focusing on providing both immediate nourishment and education, we empower our neighbors to strengthen their health and enhance recovery. To enroll in our program, a licensed medical provider must complete an application, which can be easily submitted via email or fax to our Client Services team.
