At Partners in Care Foundation, we offer an empowering Diabetes Self-Management workshop in Sylmar, CA, designed to help individuals take control of their diabetes or pre-diabetes. Participants will learn to interpret their blood sugar levels, effectively manage stress, and create a diet that works for their health. Our comprehensive program includes essential diabetes education covering healthy eating, medication management, communication skills, and more, all delivered in a 6-part workshop series consisting of 2.5-hour classes held weekly. We provide both online and in-person options, with workshops accessible in English and Spanish. While there is no fee to participate in our programs, any contributions to support our mission will be appreciated, as we strive to enhance the well-being of our community.
