Located in Vista, CA, Mama's Kitchen's Chronic Kidney Disease Pilot Nutrition Program is dedicated to improving health outcomes for individuals facing chronic kidney disease. We provide free home delivery of three nutritious meals daily, ensuring that our participants receive food tailored to their specific dietary needs. Our mission is to deliver medically tailored meals and personalized medical nutrition therapy that not only meet essential nutritional requirements but also enhance healthcare results and lower costs in just three months. To participate, a referral from a healthcare provider is required, and we regret that we cannot enroll individuals residing in facilities serving more than seven meals per week, those with limited ability to adhere to our program, those with a life expectancy of less than a year, or those with severe food allergies.
