At the Nurse Collaboration Program in Arvada, CO, we are dedicated to ensuring a healthy start for mothers and their little ones. Our initiative, part of Jefferson County Public Health, provides a variety of essential services for families with newborns and children up to age 5. This voluntary home visitation program, tailored for families referred by the Jefferson County Human Services Department, offers expert guidance and care coordination through our compassionate nurses. Each session, typically spanning 4 to 7 months, focuses on developmental screenings and personalized support for parents, grandparents, and caregivers. Ideally, families engage in at least eight meetings to maximize benefits. Our services can continue even after case closure, as we collaborate closely with caseworkers through the CAT system to ensure every child and caregiver receives the support they need during this critical time.
