Our dedicated case managers are here to guide you through every step, ensuring you have access to essential resources tailored to your needs. We provide a range of services including case management, assistance with short or long-term housing, access to nutritious foods, medical transportation, emergency financial aid for utilities and phone bills, budgeting support, Naloxone distribution, and obtaining vital documents. To begin your journey towards stability and health, simply provide proof of diagnosis to one of our case managers, either in-person or via fax. This program is available at no cost to you through the Ryan White Program, reinforcing our commitment to supporting the wellbeing of our community.
