Our dedicated case managers are here to guide you through the unique journey of managing your health and well-being. We provide comprehensive case management, assistance in locating both short and long-term housing, and access to nutritious food options. Our services also include medical transportation, emergency financial assistance for utilities and phone bills, budgeting support, Naloxone distribution, and help with vital documents. To get started, simply bring proof of diagnosis to a case manager, either in person or via fax. We aim to ensure that you receive the support you need, all provided at no cost through the Ryan White Program.
