Located in Conifer, CO, the Family Tree Homelessness Program is dedicated to providing vital housing navigation and comprehensive support services to individuals and families experiencing homelessness or at risk of becoming homeless. Our mission centers on fostering safety, stability, and self-sufficiency through affordable housing initiatives. We offer a range of essential services, including financial assistance for rent, deposits, and utilities, as well as home-based case management tailored to individual needs. We connect clients with community resources through our hotline for information and referrals, and we provide crisis assistance that includes emergency food, hygiene products, and 911 cell phones. Our program also focuses on goal setting and planning to help clients overcome barriers to self-sufficiency, while assisting them in attaining and maintaining employment and educational aspirations. Additionally, we help connect individuals to severe weather shelters, ensuring that everyone has access to the support they need during times of crisis.
