Based in Ignacio, CO, HIV Support Services through the Colorado Health Network is dedicated to empowering individuals living with HIV by offering comprehensive support tailored to their unique needs. Our compassionate case managers are here to assist you at every step, helping you gain access to essential programs based on income and residency criteria. We provide a range of vital services, including case management, support in securing short or long-term housing, access to nutritious foods, medical transportation, emergency financial assistance for utilities and phone bills, and budgeting guidance. Additionally, we offer access to naloxone and help with obtaining vital documents. To begin your journey with us, simply provide your case manager with proof of diagnosis, which can be submitted in person or via fax. Our services are free of charge through the Ryan White Program, and we're here to support you every step of the way.
