Cancer Transitions offers a vital evidence-based program designed specifically for survivors of all types of cancer who have recently completed treatment. Located in Wallingford, CT, our mission is to empower individuals in their survivorship journey, particularly those who have finished treatment within the past 24 months, although our resources are beneficial for survivors at any stage. We provide essential services including nutritional support, personalized exercise plans, emotional health education, and medical management guidance. Through our supportive program, we aim to enhance the quality of life and foster the well-being of cancer survivors in our community.
