Located in Cheswold, Delaware, Cancer Transitions offers a compassionate, evidence-based program designed for cancer survivors who have recently completed treatment. Targeted primarily at those who are within 24 months of finishing their treatment, our initiative is also valuable for individuals further along in their survivorship journey. We empower participants through a holistic approach, offering nutritional support, customized exercise plans, emotional health education, and guidance on medical management. Our mission is to support cancer survivors in reclaiming their health and well-being as they navigate life after cancer.
