Located in Laurel, DE, Cancer Transitions is a compassionate and evidence-based program designed to support cancer survivors who have recently completed their treatment. While primarily focused on those who finished their journey within the last 24 months, this program is also open to survivors at any stage of their recovery. Participants can access vital resources including nutritional guidance, customized exercise plans, emotional health education, and medical management support. Our mission is to empower cancer survivors on their path to a healthier, fulfilling life beyond treatment, fostering resilience and well-being within our community.
