Cancer Transitions is a compassionate, evidence-based program in Lewes, DE, designed to empower cancer survivors who have recently completed treatment. Specifically focused on individuals who have finished their treatment within the last 24 months, the program is open to anyone in their survivorship journey, regardless of when they completed treatment. We provide essential support in nutrition, personalized exercise plans, emotional health resources, and guidance on medical management, all aimed at fostering holistic well-being for survivors as they navigate life post-treatment.
