About Children with Special Health Needs Program (CSHNP)
The Children with Special Health Needs Program (CSHNP) in Hanamaulu, Hawaii, is dedicated to ensuring that every child and youth with special health care needs, up to 21 years old, achieves optimal health, growth, and development. Our mission focuses on supporting children with chronic medical conditions who require specialized care, alongside their families. CSHNP provides a range of essential services, including information and referrals, service coordination for medical specialty care, nutrition guidance, and specialized services for hearing and vision. We also facilitate transition planning to adult health care and offer access to neurology, cardiac, and nutrition clinics throughout the Neighbor Islands. Additionally, we extend limited financial assistance for specialty care as well as support for necessary medical services such as laboratory tests, X-rays, office visits, prescription medications, hearing aids, and even travel expenses for families needing medical care on different islands. Families can easily access our resources by applying directly, while health care providers can submit a Referral for Service form to connect their patients with our comprehensive support.
