The Children with Special Health Needs Program (CSHNP) in Lahaina, HI, is dedicated to ensuring that all children and youth aged 0-21 with chronic medical conditions achieve their fullest potential in health and development. Our mission is to support families navigating the complexities of long-term health needs through a range of essential services. We offer information and referral services, service coordination for specialized medical care, nutrition services, and specialized hearing and vision care. Additionally, we provide transition planning to adult health care, clinics for neurology, cardiac, and nutrition on Neighbor Islands, and limited financial assistance for families meeting income criteria. We help facilitate access to medical specialist visits, laboratory tests, X-rays, EKGs, EEGs, prescription medications, and even hearing aids. For Neighbor Island families requiring medical services on another island, we assist with travel and lodging expenses. Health care providers can easily refer patients to our program, while families can reach out directly to apply for support.
