Cancer Transitions is a dedicated program in Mililani, HI, designed to support survivors of any cancer diagnosis who have recently completed their treatment. Our evidence-based approach is particularly focused on those who finished treatment within the last 24 months, although we welcome survivors at any point in their journey. We offer a comprehensive range of services, including nutritional support to help rebuild strength, customized exercise plans to enhance physical well-being, emotional health education to foster resilience, and medical management support to navigate ongoing health needs.
