Cancer Transitions is a compassionate program based in Ookala, HI, dedicated to supporting cancer survivors who have recently completed treatment. Designed particularly for those who have finished their treatment within the last 24 months, our program also welcomes survivors at any stage of their recovery journey. We provide essential resources that focus on nutritional support, customized exercise plans, emotional health education, and comprehensive medical management assistance. Our mission is to empower survivors with the knowledge and tools needed for a healthy and fulfilling life beyond cancer, fostering a community of hope and resilience.
