The Community Home Visitation Program is committed to enhancing the health and well-being of pregnant women, children, and families in Des Moines, IA, and Polk County. Our dedicated team, which includes Registered Nurses, a nutrition counselor, and a Social Worker, provides personalized home visits tailored to the unique needs of each family. Our comprehensive services encompass prenatal and post-partum health education, child development screenings, perinatal and caregiver depression screenings, as well as referrals to vital community resources and nutrition counseling. We aim to support families every step of the way, ensuring they receive the essential care and guidance they require. Referrals are welcome from physicians, social workers, family members, friends, or through self-referral. We also accept Medicaid to make our services accessible to all families.
