The Wellness Project, located in Des Moines, IA, is dedicated to enhancing the lives of individuals in Polk County by providing targeted case management and integrated wellness services. Our mission is to support individuals with Intellectual Disabilities, Developmental Disabilities, and Brain Injury Waiver eligibility by fostering recovery and resilience through access to health promotion opportunities. We offer a variety of services, including Community Living, Employment and Education Supports, and recreational activities designed to promote emotional and physical well-being.
