Located in Cedar Lake, IN, the Program of All-Inclusive Care for the Elderly (PACE) is dedicated to providing comprehensive medical and supportive services tailored to seniors with chronic care needs. Our mission is to empower older adults to maintain their independence in the comfort of their own homes for as long as possible. PACE offers a wide range of essential services, including adult daycare, nutrition counseling, various therapies, meal preparation, medical care, dental care, vision care, in-home assistance, and coordinated hospital and nursing home care. While all participants must be eligible for nursing home care, only a small percentage, approximately seven percent, actually reside in such facilities. PACE is committed to ensuring seamless care for all enrollees, with the program covering nursing home expenses as needed and continuing to oversee their overall well-being. Our approach is supported through Medicare and Medicaid reimbursement, ensuring that eligible individuals receive the care they deserve.
