At the Program of All-Inclusive Care for the Elderly (PACE) in Crown Point, IN, we are dedicated to enhancing the quality of life for seniors with chronic care needs by providing comprehensive medical and supportive services that foster independence in the comfort of their own homes. Our holistic approach addresses a wide range of essential services, including adult daycare, nutrition counseling, various therapies, meals, medical, dental, and vision care, as well as in-home support and transportation. While PACE participants are certified for nursing home care, our aim is to keep approximately 93% of enrollees living at home. Should nursing home care become necessary, our program seamlessly covers those needs while ensuring continuous care coordination.
