The Program of All-Inclusive Care for the Elderly (PACE) in Plymouth, IN, is dedicated to providing comprehensive medical and supportive services that empower seniors with chronic care needs to maintain their independence at home for as long as possible. Our program encompasses a wide range of essential services, including adult daycare, nutrition counseling, therapeutic support, meal preparation, and various medical and dental care options. We work closely with our participants to ensure they receive coordinated in-home care, and we also provide necessary hospital and nursing home care when required. While participants must be certified as needing nursing home care to enroll, approximately 93 percent of our PACE members live in their own homes.
