The Program of All-Inclusive Care for the Elderly (PACE) in South Bend, IN, is dedicated to empowering seniors with chronic care needs by providing a full range of medical and supportive services that enable them to maintain their independence at home for as long as possible. Our comprehensive offerings include adult daycare, nutrition counseling, various therapies, meals, medical care, dental and vision care, in-home care, and transportation services. While certification for nursing home care is required for enrollment, only a small fraction of our participants actually reside in nursing facilities, as we prioritize home-based care. Should nursing home care become necessary, PACE ensures that costs are covered while continuing to manage and coordinate the ongoing care of our enrollees.
