Cancer Transitions is a supportive program based in Fort Knox, KY, designed for cancer survivors who have recently completed treatment, specifically those within the last 24 months. Our mission is to empower individuals on their journey to recovery by offering comprehensive resources that include nutritional support, customized exercise plans, and essential education on emotional and medical management. While our program is primarily focused on recent survivors, we are committed to assisting individuals at any point in their survivorship. Through our evidence-based approach, we strive to enhance quality of life and foster resilience in the face of cancer.
