Cancer Transitions, located in Jackson, KY, is a compassionate and evidence-based program designed specifically for survivors of any cancer diagnosis who have recently completed their treatment. While primarily targeted at those within 24 months post-treatment, our resources are beneficial for survivors at any stage of their journey. With a focus on holistic well-being, we provide essential support in areas such as nutritional guidance, personalized exercise plans, emotional health education, and medical management resources.
