The Living with Type 2 Diabetes Program in Liberty, KY, is a complimentary initiative designed for individuals recently diagnosed with type 2 diabetes. Over the course of 12 months, participants will engage in a supportive learning journey that includes six informative e-booklets tailored to address their needs, along with a monthly e-newsletter offering seasonal tips and local community event information in both English and Spanish. Enrollees are also encouraged to submit their phone numbers to participate in the monthly 'Ask the Experts' Q&A series, providing access to American Diabetes Association specialists for personalized advice, insights on the connection between diabetes and heart health, and exciting giveaways. Our mission is to empower individuals to manage their diabetes effectively while fostering a sense of community and support.
