Located in Stoneham, MA, the Neighborhood Program of All-Inclusive Care for the Elderly (PACE) is dedicated to empowering older adults to maintain their health, independence, and vitality in the comfort of their own homes. Our community-centered model offers a comprehensive array of services including integrated medical care, nutritional counseling, personalized fitness plans, and wellness workshops. We also facilitate transportation to medical appointments and provide meal delivery, ensuring that our participants have access to essential resources. With a full interdisciplinary team, we offer a medical clinic, rehabilitation gym, therapeutic recreation, and a range of supportive services such as homemaker assistance, dental care, and pharmacy services. We welcome participants covered by MassHealth and Medicare, and are here to support you on your journey to well-being.
