Living Well with Chronic Disease is a dedicated program based in Columbia, MD, offering a free six-week self-management initiative designed to empower individuals in managing chronic health conditions. Our program focuses on providing essential education and practical self-management skills, helping participants navigate the challenges of chronic illness with tools to cope with frustration, fatigue, and pain. Through collaborative discussions in small groups, participants learn to effectively communicate with family, friends, and healthcare providers, while also engaging in exercises that enhance strength, flexibility, and endurance. We emphasize the importance of nutrition and guide individuals in developing healthy eating habits.
