The DHHS Community Care Program, part of the Office of Population Health Equity located in Alfred, ME, is dedicated to helping Mainers overcome barriers to accessing essential health programs and social services. Through our user-friendly online referral system, we connect individuals and families with a wide range of resources tailored to their needs, including culturally and linguistically appropriate services, maternal and child health support, and healthcare navigation assistance. Our offerings also encompass health and wellness education, as well as options for food, heating, and transportation resources. We prioritize mental health by providing support for stress and anxiety, along with COVID-related assistance. By simply submitting a referral form, households can access the necessary help, and within 1-3 days, one of our partner organizations will reach out to discuss available options and services based on the specific needs outlined.
