The DHHS Community Care Program, part of the Office of Population Health Equity at ME CDC, is dedicated to assisting residents of Auburn, ME, and beyond in navigating essential health and social services. Our online referral system is designed for all Mainers, especially those facing barriers to access. We connect individuals and families to vital resources ranging from culturally and linguistically appropriate services to support for mothers and children, along with healthcare navigation assistance, health education, and essential needs like food, heating, and transportation. Additionally, we offer support for mental health, including stress and anxiety management, as well as COVID-related resources. By completing our referral form, households can receive tailored connections to organizations that will reach out within 1-3 days to discuss specific needs and options. Each of our partner programs has its own eligibility criteria, ensuring that we provide the most relevant support to our community.
