The DHHS Community Care Program, part of the Office of Population Health Equity at the ME CDC in Bath, ME, is dedicated to helping Mainers overcome barriers to accessing vital health and social services. Utilizing a streamlined online referral system, we connect individuals and families with essential resources tailored to their needs. Our program welcomes everyone and offers a diverse range of assistance, from culturally and linguistically appropriate services to support for mothers, babies, and children. We help navigate the healthcare landscape and provide education on health and wellness, as well as options for food, heating, and transportation resources. Additionally, we address stress and anxiety support and offer COVID-related resources. To get started, simply fill out our referral form; our staff will ensure you are connected with a partner organization within 1-3 days based on your specific needs. Please note, eligibility varies by program and region, and we encourage submissions for each household.
