The DHHS Community Care Program, part of the Office of Population Health Equity in Belfast, ME, is dedicated to helping Mainers overcome barriers to accessing essential health programs and social services. Through our online referral system, we connect individuals and families to a wide range of resources tailored to their needs, including culturally and linguistically appropriate services for mothers, babies, and children, navigation support for the healthcare system, health and wellness education, and options for food assistance, heating, and transportation. We also offer valuable support for stress, anxiety, and COVID-related challenges. By submitting one referral per household, community members can receive timely assistance from our partner organizations, typically within 1-3 days. We recognize that each program has unique eligibility requirements, and we work diligently to connect you with the right resources available in your region.
