The DHHS Community Care Program, part of the Office of Population Health Equity in Berwick, ME, is dedicated to ensuring that all Mainers have access to vital health programs and social services. By filling out our referral form, community members can initiate a connection to multiple organizations tailored to their needs, with outreach typically occurring within 1-3 days.
