The DHHS Community Care Program, part of the Office of Population Health Equity in Biddeford, ME, is dedicated to connecting all Mainers with essential health programs and social services aimed at overcoming barriers to access. Our user-friendly online referral system allows individuals and families to easily explore a variety of resources, including culturally appropriate services, support for mothers and children, and guidance through the healthcare system. We also provide connections to food assistance, heating, and transportation resources, alongside wellness education and support for managing stress and anxiety, including COVID-related resources. To get started, submit a single referral per household, and within 1-3 days, one of our partner organizations will reach out to discuss your needs and available options, keeping in mind that eligibility may vary by program and region.
