At the DHHS Community Care Program, part of the Office of Population Health Equity in Brunswick, ME, we strive to empower Mainers by connecting them with vital health programs, social services, and essential resources. Our online referral system is accessible to everyone, especially those facing barriers in accessing the support they need. By completing our referral form, individuals gain access to a range of services tailored to their unique needs such as culturally appropriate assistance, resources for mothers and children, healthcare navigation, health and wellness education, and options for nutrition, heating, and transportation. We also provide support for stress and anxiety, as well as COVID-related resources to address current challenges. Our dedicated staff will collaborate with our network of partners to connect individuals with the appropriate resources within 1-3 days of referral submission, ensuring a compassionate approach to meet each person's needs. We invite community members to submit one referral per household to begin their journey toward better health and support.
