The DHHS Community Care Program, part of the Office of Population Health Equity in Buxton, ME, is dedicated to ensuring that all Mainers have access to essential health programs and social services, especially those facing barriers. Through our user-friendly online referral system, individuals can connect with a wide range of resources tailored to their specific needs, including culturally and linguistically appropriate services, support for mothers and children, healthcare navigation assistance, health and wellness education, and resources for food, heating, and transportation. Our compassionate team is here to address stress, anxiety, and provide COVID-related support. After submitting a referral form, one of our partners will typically reach out within 1-3 days to discuss options and next steps, keeping in mind the unique eligibility criteria and regional differences in available resources.
