The DHHS Community Care Program, part of the Office of Population Health Equity in Camden, ME, is dedicated to helping residents access vital health programs, social services, and an array of resources tailored to their unique needs. Our warm and inclusive approach ensures that everyone can benefit from assistance, especially those facing barriers to utilizing available resources. Through our online referral system, individuals and families can easily connect to culturally and linguistically appropriate services, support for mothers and children, healthcare navigation assistance, health and wellness education, and vital resources for food, heating, and transportation. We also provide resources for managing stress and anxiety, as well as COVID-related support. By submitting a single referral form per household, our dedicated staff will streamline the process and connect applicants with the right organizations within 1-3 days, based on eligibility and regional resources.
