The DHHS Community Care Program, part of the Office of Population Health Equity in Camden, Maine, is dedicated to ensuring that all Mainers have access to essential health programs and social services, especially those facing barriers. Through our user-friendly online referral system, we connect individuals to a wide range of resources tailored to their unique needs, including culturally appropriate services, maternal and child support, healthcare navigation assistance, health and wellness education, and vital resources for food, heating, and transportation. We also offer support for stress and anxiety, as well as information related to COVID-19. Each household can submit one referral, and our compassionate staff will work diligently to link you with the appropriate organizations within 1-3 days, ensuring you receive the support you deserve.
