The DHHS Community Care Program, part of the Office of Population Health Equity in Cape Elizabeth, ME, is dedicated to helping residents connect with vital health programs and social services throughout the state. Our online referral system is designed to assist everyone, particularly those facing barriers in accessing eligible resources. By filling out our simple referral form, community members can gain access to a wide range of services, including culturally and linguistically appropriate support, resources for mothers and children, assistance with the healthcare system, health and wellness education, and options for food, heating, and transportation. We also offer support for stress and anxiety, as well as COVID-related resources. After submitting a referral, our compassionate staff will work to connect you with partner organizations within 1-3 days, ensuring that you receive the support you need based on your individual circumstances. Please note that eligibility for programs may vary by region, and we provide one referral per household to facilitate this connection.
