The DHHS Community Care Program, part of the Office of Population Health Equity in Cumberland Center, ME, is dedicated to bridging the gap between Mainers and vital health and social services. With a user-friendly online referral system, we assist individuals facing barriers to accessing the resources they need, ensuring that everyone can find the support they deserve. By filling out our referral form, community members can connect to a wide range of services, including culturally and linguistically appropriate programs, healthcare navigation assistance, health and wellness education, and resources for food, transportation, and stress relief. We also provide targeted support for mothers, babies, and children, along with COVID-related resources. Each referral is personalized, with our caring staff reaching out within 1-3 days to discuss needs and options, all while considering regional eligibility requirements. Together, we aim to empower individuals and families in our community to lead healthier, more fulfilling lives.
